On fol. 1v: dedication headed "Octavianus Petrutius forosemproniensis Hieronymo Donato patricio Veneto Felicitatem," and signed and dated "Venetiis decimo octavo cal. iunias. Salutis anno. MDI." On fol. 2: a letter from Bartolomeo Budrio to Girolamo Donato, headed "Bartholomaeus Budrius Justinopolita. Hieronymo donato patricio Veneto .S."
